NIAGARA FALLS - Niagara Falls Memorial Medical Center’s program to support family members caring for Medicaid beneficiaries is receiving a $376,000 grant from the state Department of Health.

The program, known as Building Bridges to Home and Community Care, involves restructuring the hospital discharge process to ensure better post-hospital care and reduce readmissions and nursing-home stays, the hospital said in a news release.

Led by a team of specially trained staffers from Memorial and Horizon Health Services, the hospital hopes to generate more than $2.3 million in Medicaid savings by September 2015.
