Name of product: Children’s Hooded Fleece Sweat shirts Units: About 2,300. Distributor: La Jolla Sport USA of Irvine, Calif. Hazard: The recalled sweat shirts have a drawstring through the hood, which can pose a strangulation hazard to children. In February 1996, CPSC issued guidelines to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets or sweat shirts. Incidents/injuries: None reported. Description: The recall involves sweat shirts with style names “Love Goes” (style 39810012) and “Up and Under” (style 39202202). They were sold under the O’Neill brand name in child sizes S to XL. The RN No. 85773 is printed on the care label in the neck. Sold at: Ron Jons’ Surf Shop, Dillards and McCaulous stores from June through August for about $65. Manufactured in: China. Remedy: Remove the drawstrings to eliminate the hazard and return the garment to the place of purchase or to the firm for a full refund. Animal Masks

Name of product: Children’s Animal Masks and Pendants Units: About 1,400 pieces. Manufacturer: Team Work Trading of Los Angeles. Hazard: The animal masks and pendants contain high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects. Incidents/injuries: None reported. Description: The mask resembles the face of a cat. The pendants have various animated symbols including a dog tag with fire symbol (Bleach), knife and lion symbol (Final Fantasy), Mickey Mouse symbol (Kingdom of Hearts), and a burning sun symbol (Naruto). The pendants were sold with silver-colored chains. Sold at: Gift shops and modeling stores nationwide, and the Team Work Trading store from November 2008 through March for between $4 and $8. Manufactured in: China. Remedy: Stop using the recalled items and contact Team Work Trading for a refund or exchange. Consumer contact: Call collect at (213) 680-4489 from noon to 8 p. m. Office Chairs

Name of product: Office Max Task Chairs Units: About 216,000. Importer: OfficeMax Inc. of Naperville, Ill. Hazard: The back and the base post of the chair can break while in use, posing a fall hazard to consumers. Incidents/injuries: OfficeMax has received about 35 reports of the chair backs or posts breaking, including 15 reports of injuries involving lacerations, muscle strains, contusions and concussions. Description: OfficeMax Task Chairs with model numbers OM182 and OM96614. The model number is located under the chair’s seat on a white UPC label. The chairs are charcoal or dark charcoal in color and have black plastic arms and a rolling plastic and metal base. Sold at: OfficeMax, on the firm’s Web site www.officemax.com, in OfficeMax catalogs and through direct commercial sales to businesses from September 2003 through July 2008 for between $40 and $65. Manufactured in: China. Remedy: Stop using the chairs and return them to any OfficeMax store for a full refund or a $55 gift card if the consumer does not have a receipt.
