A $250,000 federal grant will provide for daily monitoring of an additional 75 diabetes patients in their homes in rural Chautauqua and Genesee counties for the next year, Reps. Kathy Hochul, D-Hamburg, and Brian Higgins, D-Buffalo, announced this week.
The grant comes with a recommendation to extend it for three more years. The grant will support daily telephone checkups by nurses from the Visiting Nursing Association of Western New York. The nurses will report to physicians, help the patients manage their conditions and reduce the patients’ need for expensive emergency room visits.
Last year, the VNA provided similar services to nearly 500 urban and rural Western New Yorkers.
